SURVEY QUESTIONNAIRE
This adapted survey questionnaire is utilized to determine the social support and
self-efficacy on the learning skills among college students. Kindly follow the directions
provided for each part of the research instrument. Your honest answer to each
statement will help the researcher to come up with a sound result to refine the college
instructional programs and initiatives. Confidentially and anonymity of your responses
shall be guaranteed with stringent observance on the rule of Data Privacy Act R.A.
