Physical and Mental Wellness Weekly Reports, Exams of German Language

A series of weekly reports on the physical and mental wellness of students, as well as their plans for improving their wellness in the following week. The reports include self-assessments of physical and mental wellness on a scale of 1 to 10, as well as reflections on activities and plans for future improvement.

- Your ``Week 3`` report will be completed after your second Phys. Ed. Class; reflect on your past
week and your plans for the upcoming week and submit at the beginning of class in week 3.
- The Weekly reports are submitted every week, at the beginning of class
- The teacher will return the previous weekly report to you at the beginning of class the following
week (Week 3 will be returned to you at the start of class during week 4, etc.).
- If you are absent from class, you will not receive any marks for that week
- All 10 weekly reports need to be signed by your teacher

Week 5 Teachers’ initial: __

On a scale of 1 (poor) to 10 (excellent) my PHYSICAL wellness last week was I felt this way because? I felt well inside my body after we had done handball and soccer and I had a lot of energy and had a great time. Plus the day before i had done badminton so i felt even better. I am going to positively influence my PHYSICAL wellness this week by…. going to positively influence my physical wellness this week by walking a lot between my courses. Plus I walk everyday down to my mailbox to get my taxi almost everyday. I will also try to stay active before and after my phys ed class is done

Update on Fitness test progressions: I think I still need to work on the plank and the push ups and my

possessions. On Tuesday I practiced my push ups and planks. On a scale of 1 (poor) to 10 (excellent) my mental wellness last week was I felt this way because? I’ve had a lot of headaches lately and not enough sleep and had a very low appetite. But today I've recovered enough sleep and am ready to do sport and try my best and stay focused.

Week 11 Teachers’ initial: __

On a scale of 1 (poor) to 10 (excellent) my PHYSICAL wellness last week was I felt this way because? 8. I felt very good yesterday since I pushed myself a whole lot for badminton and didn’t have a long break and when I got home I immediately fell asleep. So that felt incredible. Today i will also push myself forwards and whatever you have planned for us. I am going to positively influence my PHYSICAL wellness this week by I worked a little on it, by walking up my hill to go see the eclipse on monday and my dad joined me but went by car since he has a terrible back. The eclipse was spectacular. We saw the darkness fall and we could see Mount Orford which was amazing!

On a scale of 1 (poor) to 10 (excellent) my ____Enviromental____________ wellness last week was I felt this way because? i was a 6 since i did see the eclipse and walked and my trail was like in the forest and its uphill a lot which is tough, but i got to see something rare and special so in the end it felt amazing and my dad brought camping chairs and drinks. So after that big exercise I could finally relax a little.
