Section A – Safety and Risk Management

  1. a. Explain in detail the three main reasons for accident prevention. (10) b. What are the three main causal factors for accidents? ( 10)
  2. The following principles of behaviour have been identified:
    • Punishment
    • Negative reinforcement
    • Recency effects and habit change
    • Expenditure of effort
    • Modelling/imitation effects
    • Discrimination learning and the stimulus control of behaviour
    • Positive reinforcement
    • Extinction Explain four of these principles, using examples to demonstrate your answer. (4x5)
  3. a. Differentiate between an accident and an incident b. How can the recording of incident data improve safety at work? c. What difficulties might you encounter in incident recording? d. How might you overcome these difficulties? (4x5)
  4. Perform an accident root analysis of the following: a. An employee was walking on a floor b. He tripped over a hose pipe c. He fell, striking his head against a table In your answer, consider both short/medium and long term solutions. (20)

  1. Critically discuss the role and powers given to the Health and Safety Authority, and discuss whether in your view they are effective in the enforcement of the provisions of the Safety, Health and Welfare at Work Act
